If you are a part time student (enrolled in 6-11 credits), you may complete and submit this form AFTER filing and receiving acknowledgement that your 2012-13 FAFSA has been processed, and AFTER applying for a New York State TAP award at www.tapweb.org. Awards depend upon the amount of funding available from New York State, and all applicants may not receive an award.

**Independent Students - Were required to file the FAFSA using their and spouse's (if married) financial information.
