Proof of ImmunizationsNew York State Public Health Laws require all students attending college to provide proof of immunity to measles, mumps and rubella, to be informed of the risks of meningococcal meningitis and given the option of being immunized. If you were born prior to 1957, you do not need to provide proof of immunity to measles, mumps and rubella. If you have contraindications to immunization or have genuine and sincere religious beliefs contrary to immunization, you will need to contact Student Central for a waiver. Students must also complete the meningitis form. Please print the Certificate of Immunization, have your health care provider complete it, and return it to Student Central.
